Your provider checklist
The checklist saves only twelve checked or unchecked answers under caredaybook-provider-checklist-v1 in this browser. It has no fields for personal or health information, and its script does not transmit your answers. Reset removes this entry only. Export downloads a plain-text file onto your device, and printing uses your browser’s print dialog.
Your planner
The care planner saves its fields in this browser’s local storage so that your numbers remain available when you return. They are not transmitted by the planner to Care Daybook. The reset button removes the saved planner entry. Avoid storing sensitive health information or personal identifiers.
Website requests
The website’s hosting infrastructure may receive standard request information, such as IP address, requested page and browser information, to serve and secure pages. We use Google Analytics to understand page readership. Analytics cookies are enabled unless you decline; advertising storage and advertising personalization are denied. You can accept or decline in the notice and change your choice through Analytics preferences in the footer. Declining disables our analytics collection and clears Google Analytics cookies for this site. We send page paths without query strings or fragments, and do not send planner values, notes or search input. Google may process technical information associated with website requests. Fonts and illustrations are served locally.

External services
When you follow a provider link, the destination’s privacy policy applies. Care Daybook does not control that service’s forms, payments or handling of health information. Browser settings can also clear local website storage.
