Collect your existing records
Ask your current service how to obtain visit records, prescription information and any active insurance authorization details. A new clinician may need a fresh assessment. Do not assume that a prescription, authorization or payment arrangement transfers automatically.
Confirm who owns each next step
Establish which team requests records, manages insurance work and communicates with the pharmacy. Tell the new clinician about your existing care and ask them to guide the transition. Avoid independently changing treatment to fit a shipment schedule.
Close the billing loop
Review old and new renewal dates before paying for overlapping programs. Keep written cancellation confirmation and ask what happens to orders already in progress. Your medical handoff and subscription cancellation are separate tasks.
Use these questions with a licensed clinician. Availability, suitability and price depend on your circumstances.
